I mean $&@&@ $*C$&%(K CH$#@), this happens so often for me. Much more so then that glitch where you would get stuck in people. I mean, what the hell man? I've played in 24 player servers and 32 player servers, my ping hangs at around 70, as does my opponent. But it still continues to happen. I just had a back stab chain completely ruined when I stabbed a Heavy 2 FREAKIN' TIMES directly dead center in the back, and it didn't register!
What the crap? This seems to happen at least 1 per 20 back stabs. What is this? Can Valve not fix this character, are they this incompetent? I don't mean to come off as ungrateful, but this is ridiculous. No other class has to deal with this none sense. Can you imagine as a Sniper, that you pull off a full charged head shot on a Demo, and it doesn't register? Or playing as a Scout, and your shotgun blast didn't register? No, there would be massive complaints and a quick fix of this issue.
What is going on at Valve, man?
I will email them constantly about this issue until it is resolved, and everyone here who plays Spy should do the same. I'm sorry, I love the game and everything, but I've had it with this annoying bug.

Dead babies.
lots and lots of dead babies.

This annoys me too, greatly >_>. The other day I kept on forgetting what team I was on (I was red), and when I saw an blue engineer with his back to me, I stabbed him square in the back. It was the backstab animation and everything, and he didn't move at all during it. But it didn't hit, so for a second I thought "Oh, I must be on the blue team, silly me!" Then I saw that my base was colored red, and went back and stabbed him, this time it worked.
They probably know about the problems already though, it's just that the fix may not be as simple as you think.

It happens whenever you get a perfect opportunity to backstab a lot of people. I think valve just hates spies. As for me, I don't play spy. While I sympathize with all the spies who have to deal wtih this, I must say being on the recieving end of this glitch is nice, even if it isn't very fair. Getting backstabbed by spies makes me so sad, .

If you didn't know this does happen to snipers but not quite as much this happends to me the most, this is more frustrating because you see them take the headshot and the blood and everything then it didn't register, thats how tf2f best sniper snuts get away from me.

The easiest way to fix some of the problems would be to remove the long stab animation completely since all it does is cause problems and skilled players can get around it anyway.

Stabs miss due to player lag/ some other things I forget. But all online games have those issues, and TF2 actually deals with them pretty well. When Cascade went spy he said a whole lot about it.

Basic principle: - If you get the "lift" animation, you must END that animation facing your target, within melee range of your target, and the target must not be directly facing you. Any other angle than directly facing should still score your stab.
That said, lag compensation introduces complications, summarised below:
These factors make your backstab less likely to register: - Your target has a high ping (> 100) - Your target is moving forwards, away from you - Your targets is moving sideways, perpendicular to you - You are moving sideways, strafing in behind your target's back - You are moving backwards (duh)
A common way to screw up the stab is to run at their back, get the lift, and then run on past them, so you don't end the lift still facing your target.
Your ping has a small affect but is not too significant, I didn't have a problem landing backstabs even with 150 ping.
You are more likely to land a stab if you can avoid the lift altogether. To do this, begin your stab when you're not in backstabbing range, or facing a target; and end your stab when you are facing a target's back. You can increase the chance of this by looking sideways before you stab, jumping and looking up before you stab, or beginning the stab when you're not yet in range.

An inexperienced spy once never landed a back stab, upon his death, a spirit was born out of his rage that haunts butterfly knives to this day. Forever it curses who ever it can to a short life of fruitless stabbing.

One time I was backstabbing a moronic Heavy who was facing his back towards me, only to have the backstab land perfectly but not register. His back was still turned to me, so I tried again, only to have the backstab fail once again. I was standing there, frantically hacking away at his back, having each one bounce off harmlessly. I rotate to his side and score a sidestab. Nonsensical.
